PRIME-5 Revised Screener for Early Psychosis

The PRIME-5 is a brief, five-item screening tool used to identify early or subthreshold psychosis-spectrum symptoms in youth and young adults. Derived from longer psychosis-risk instruments, it helps primary care and mental health providers determine if a patient needs a comprehensive clinical evaluation.
