Disability Awareness: Innovations in Defragmenting Mental and Physical Health Care for People with Disabilities

UPMC Western Psychiatric Hospital Presents: This conference is about learning the value of community and academic partnerships to forge learning and collaborative care opportunities that benefit our most vulnerable patients. Participants will learn ways that professionals can foster moving research and best practice ideas to their clinical work. We will review the importance of learning about community resources, relevant advocates and accommodations that are needed to meet the needs of people with mental health, physical and sensory differences.

We are pleased to offer this educational activity with the support of UPMC, community, and university.

Program Overall Educational Objectives
At the conclusion of this conference, participants should be able to:
• Describe models to improve health and quality of life outcomes for people with disabilities that include social determinants of health
• Discuss the possible role of virtual platforms including Artificial Intelligence to promote good health outcomes in people with complex health and mental health care needs
• Compare and contrast several models of excellence for the management of complex diseases where integrated care is integral to recovery-oriented goal planning
• Review strategies to integrate input from the patient and family about existing services for people with complex needs to promote system change
• Discuss strategies where providers can collaborate with insurers and advocates on behalf of patients with unmet mental health needs
• Raise awareness about what professional networks exist to promote community based participatory research for disability communities

Who Should Attend:
Students, advocates, mental health and physical health providers, payors, case managers and stakeholders who are passionate about the care of people with disabilities.
